Application Handbook Section 11

On-line Application Forms

The on-line application system also allows you to apply for student teaching electronically. To apply for student teaching, you will need to complete all screens. The screens are chronologically linked, which means that you must complete and submit the first screen before you can access to the second, you must complete the second before the third, etc. Your on-line application will not be "sent" to our office until all four screens have been completed and submitted, which means no placement activity can occur for you until all of these electronic forms have been completed. If you are in a special program, which does not include listing your preferences, you will have fewer screens to go through before your application is submitted.

If you are not yet admitted to the Teacher Education Program (TEP) you will not be able to access the Student Teaching section. Contact Jane Benjamin at 856-8523 or jabenjam@indiana.edu if you have questions about admittance to TEP.

On-line Application Form #1: Demographic Data

This form asks you to provide us with your emergency address, which must be tabbed. This information will become part of our database and used to contact you with information or questions about your placement. It is critical, therefore, that you keep this form updated.

On-line Application Form #2: Placement Details

This form asks you to indicate exactly which semester and year you will be student teaching; what your licensure/endorsement areas are; whether you are in a special program; and other general information that will aid us in placing you.

BE SURE THIS FORM IS COMPLETED ACCURATELY AND COMPLETELY. ONCE SUBMITTED, YOU CANNOT ACCESS THIS FORM AGAIN. ANY CHANGES THAT YOU NEED TO MAKE WILL HAVE TO BE DONE THROUGH OUR OFFICE.

On-line Application Form #3: Site Preferences

This can be a challenging form to complete. To help you in completing this form, you should have read all placement and site information from Section 10, and completed the site selection worksheet in that section, and perhaps have accessed the Indiana School Directory.

Our office will mail your materials to school systems according to the information you provide, so be careful. Once your application has been forwarded to a school, we cannot change the request until we hear from school officials.

On-line Application Form #4: Student Teaching Contract and Signature Form

This form gives the Office of Student Teaching the authority to release your transcript(s) to school systems. There are two versions of this form-- the on-line application and the other in hardcopy (frequently called "the pink form"). We must have both the hardcopy as well as the online version because we must have an actual signature in order to release your transcript. You will receive the pink form during your application meeting and you will turn it in when you submit your paper application.

If you attended schools other than IU, you must submit transcripts from all previous institutions unless that course work appears on your IU transcript. In most cases, undergraduate courses transferred (from other institutions) to IU should appear on your IU transcript. Graduate students will have to submit transcripts from their undergraduate institutions

ALL FOUR FORMS MUST BE COMPLETED BEFORE WE CAN BEGIN WORKING ON YOUR PLACEMENT.
